Ön Çapraz Bağ Kopuklukları
Ön çapraz bağ nedir?
Köpeklerdeki kranial çapraz bağ (CrCL), insanlardaki ön çapraz bağ (ACL) ile aynıdır. Femur ile tibia arasında bağlantı kuran sert lifli, şerit şeklinde bir dokudur ve tibianın femur üstünden öne doğru kaymasını önler. Bu önleme diz ekleminin stabil halde çalışmasına katkı sağlar. Ayrıca diz ekleminin aşırı açılanmasını veya dönmesini önlemeye yardımcı olur.
İnsanlarda genellikle spor aktiviteleri esnasında ön çapraz bağ aniden kopar fakat köpeklerde bağ, aşırı travma nedeniyle aniden kopmak yerine, genellikle zamanla yavaşça dejenerasyona uğrar, tıpkı yıpranan bir ip gibi. Bu önemli fark, köpeklerde çapraz bağ yaralanması için önerilen tedavi seçeneklerinin, insanlar için önerilen tedavi seçeneklerinden bu kadar farklı olmasının başlıca nedenidir.
Köpeklerin büyük çoğunluğunda, ön çapraz bağ uzun vadeli dejenerasyon sonucu yırtılır ve bu da bağ içindeki liflerin zamanla zayıflamasına neden
olur. Bunun nedeni kesin değildir ancak genetik faktörler muhtemelen en önemli sebeptir. Genetik faktörlerin sebep olduğunu düşünmemizin sebebi,
bazı ırklarda çok daha fazla ön çapraz bağ yaralanmasına rastlamamızdır. (Labradorlar, Rottweilerlar, Boxerlar, West Highland White Terrier'ler ve
Newfoundland'lar).
Topallama, ön çapraz bağ yaralanmasının en yaygın belirtisidir. Bu, bazı köpeklerde egzersiz sırasında veya sonrasında aniden ortaya çıkabilir veya
diğerlerinde ilerleyici ve aralıklı olabilir. Bazı köpeklerde aynı anda her iki dizde de etkilenir ve bu köpekler genellikle yüzüstü pozisyondan kalkmayı zor bulur ve çok bozuk bir yürüyüşe sahiptir. Şiddetli vakalarda, köpekler hiç ayağa kalkamaz ve yanlışlıkla nörolojik bir sorun olduğundan şüphelenilebilir. Diğer yandan sabit ve ayakta dururken köpeğinizin parmak ucu yere değecek şekilde bacağını yarı askıda tutması tipik bir belirtidir.
Bir diz ekleminde çapraz bağ yaralanmasını takiben hemen osteoartrit başlar ve bu kireçlenme ilerleyici nitelikte olduğu için şiddeti giderek artar. Aşınmanın ileri safhalarında CrCL normal mekanik işlevini kaybeder ve ağrılı aksama, mekanik aksama ile birlikte görülür. Bu noktada, mekanik aksamayı etkileyen faktörlerden biri, köpeklerde belirgin bir şekilde geriye doğru eğimli olan tibianın (kaval kemiği) üst kısmının şeklidir. Yetersiz CrCL'ye sahip köpeklerde bu eğimin sonucu, bacak her yüklenmede femurun eğiminden aşağı doğru kayar. Bazı köpeklerde, çapraz bağ yaralanması diz eklemi içerisindeki diğer yapılara ve özellikle menisküse zarar verebilir. Tibia yüzeyine dengesi.z bir bası oluşturan femur özellikle medial menisküslerde yırtıklara sebep olabilir.

bir ortopedi cerrahı tarafından yapılan muayenede rahatlıkla koyulur. Kısmi
yırtık veya bağın erken dejenerasyonu olan köpeklerde radyografi (röntgen) veya MRI taramaları gibi diğer testler gerekebilir. Bazen de tanıyı
doğrulamak ve olası kıkırdak yırtıklarını veya diğer sorunları araştırmak için
keşif cerrahisi veya artroskopi kullanılır.
Tedavi Yöntemleri
Genel anestezi veya cerrahinin risklerinin fazla olduğu düşünülen durumlar
dışında (örneğin, ciddi kalp hastalığı, kontrol edilemeyen hormonal
bozuklukları veya bağışıklık sistemi rahatsızlıkları olan hastalar vb.) cerrahi
olmayan tedavi önerilmez. Cerrahi olmayan tedavinin temel taşları vücut
ağırlığı yönetimi, fizyoterapi, egzersiz değişikliği ve ilaçtır (antienflamatuvar
ve ağrı kesiciler). Aynı teknikler, cerrahi olarak tedavi edilen köpeklerin kısa
vadeli post-op tedavisinde de önemlidir, ancak cerrahinin birincil amacı
uzun vadeli egzersiz kısıtlaması ve ilaç ihtiyacını en aza indirmektir. 10
kg'dan büyük köpeklerin cerrahi olmayan tedaviyle klinik olarak normale
dönme şansı çok düşüktür. 10 kg'dan daha az ağırlığa sahip köpekler ve
kediler daha iyi bir şansa sahiptir, ancak iyileşme genellikle birkaç ay sürer
ve nadiren iyi sonuç verir.
Cerrahi tedaviler, hasarlı ligamenti değiştirmeyi amaçlayan teknikler ve tibiayı keserek ve diz eklemine etki eden kuvvetleri yeniden hizalayarak bağı gereksiz hale getiren teknikler olarak kategorize edilir.
İnsanlarda bağ dokusu değiştirme için kullanılan prosedürleri taklit eden çeşitli cerrahi teknikler uzun süredir uygulanmaktadır. Köpeklerde, hastanın diz çevresindeki kendi yumuşak dokuları ile çapraz bağı taklit eden yapıları bağın yerine yerleştirme teknikleri dizin fonksiyonunu normale döndürme şansı çok düşüktür. Bunun nedeni muhtemelen değiştirilen dokuların orijinal bağ dokusu kadar sağlam olmaması ve orijinal bağ dokusunun ilk etapta başarısız olmasına neden olan aynı olumsuz biyomekanik ortamda konumlandırılmasıdır. Öte yandan sentetik lifler ve bağlar, misina benzeri naylon iplikler görece daha sağlam olsa da sıklıkla diz içerisinde inflamasyona ya da aşınmaya sebep olurlar. Her ne kadar son yıllarda gelişen nano teknoloji sayesinde doku uyumlu olduğu iddia edilen sentetik lifler yaygınlaşsa da halen en geçerli ve etkili yöntemler köpeğin bir ön çapraz bağa olan ihtiyacını ortadan kaldıran biyomekanik osteotomi teknikleridir.
Bu ameliyatlar, diz ekleminin biyomekaniğini ve geometrisini, stabiliteyi korumak için ön çapraz bağa gerek kalmayacak şekilde değiştirir. Teknikte, kemiği keserek ve yeni bir pozisyonda sabitleyerek kaval kemiğinin üst kısmının yeniden şekillendirilmesini içeren birkaç varyasyon vardır. Medicapet olarak bu tekniklerin tümünü gerçekleştiriyor olabilmekle birlikte, yapılan bilimsel çalışmalar ışığında uzun vadede en sağlıklı teknikler olarak sunulan TTA ve TPLO cerrahilerini uygulamayı tercih ediyoruz.
Bu ameliyat, tibianın üst kısmında dairesel bir kesi oluşturulmasını ve kemikteki önceki eğim artık mevcut olmayana kadar plato segmentinin döndürülmesini içerir. Daha sonra kemik, kemik plakası ve vidalar kullanılarak bu yeni pozisyonda sabitlenir. Böylece femurun tibia üzerinde uygunsuz şekilde kayması engellenmiş olur. Hassas ölçümler ve kusursuz el maniplasyonu gerektirdiği için tecrübeli ve ehil cerrahlar tarafından uygulanmalıdır.
Bu ameliyat, TPLO ile aynı prensibi izler ve ön çapraz bağı gereksiz kılan bir geometrik ve biyomekanik bir değişikllik meytdana getirir. TTA'nın ardındaki
matematiksel prensipler, TPLO'nun ardındakilerden daha karmaşıktır; ancak temel prensip, quadriceps kas grubundan gelen çekiş yönünün değişmesinin, femur kemiğinin tibial plato eğiminden aşağı doğru kayma eğilimini nötralize eden diz eklemi boyunca bir kuvvet üretmesidir. Aslında, hem TTA hem de TPLO, tibial platoyu düz patellar tendona dik hale getirmeyi ve bunu yaparken femur kemiğinin tibia eğiminden aşağı doğru kayma eğilimini nötralize etmeyi amaçlar. Patellar ligament hasarı olan hastalarda TTA tekniği uygun olmayacaktır. Çünkü TTA tekniğini uygularken bu ligamentin çekme kuvvetinden faydalanılır.
Kemik iyileşmesinin bağ iyileşmesinden daha kolay ve hızlı olması sebebiyle, bu onarımlar hasarlı bağın değiştirilmesi için tasarlanan ameliyatlardan önemli ölçüde daha sağlamdır. En büyük pratik fayda, tüm köpeklerin 1-3 gün içinde ameliyat edilen uzuvda ağırlık taşımaya başlamasının beklenmesiyle, uzuv kullanımının çok güvenilir bir şekilde geri dönmesidir. TPLO ve TTA'nın mekanik avantajları, hızlı işleve dönüşle birleştiğinde, özellikle ağır köpekler, atletik hayvanlar, hafif aksama
gösteren hayvanlar ve her iki diz eklemini de etkileyen ön çapraz bağ yaralanmaları olan hayvanlar için önemlidir. Her iki dizini aynı anda etkileyen ön çapraz bağ yaralanmaları olan bazı hayvanlarda, TPLO tek bir ameliyatta her iki diz eklemine uygulanabilir. Bağ değiştirme teknikleri kullanıldığında bu mümkün değildir.
Köpeklerin %90'ından fazlası TPLO veya TTA'dan sonra normal aktivitelerine döner. Hatta her iki tekniği kedilere de başarıyla uygulayabiliyoruz. Bu operasyonlardan sonra kedi ve köpeklerin devam eden ilaç tedavilerine gerek kalmadan sınırlandırılmamış egzersize dönmelerini bekliyoruz. TPLO ve TTA için başarı oranları birbirine çok benzerdir.
Deneyimli cerrahlar çapraz bağ onarım ameliyatı yaptığında komplikasyon oranları düşüktür. En yaygın iki komplikasyon enfeksiyon ve mekanik komplikasyonlardır. Enfeksiyon antibiyotiklerle tedavi edilir. Hayvanların büyük çoğunluğunda, implantlar ömür boyu yerinde kalır ve hiçbir soruna neden olmaz. Mekanik komplikasyonlar genellikle kemikler iyileşmeden önce çok fazla egzersiz yapan köpeklerde görülür. Birçok mekanik komplikasyon sadece istirahatle giderilebilir ancak bazı sorunlar cerrahi revizyon gerektirir.
